1. Contact the REI Health Guide at 1-800-451-2967 to confirm your eligibility.
2. Work with your physician to obtain pre-certification before utilizing services.
   • Infertility is covered in network only. Log in to your member website, complete the registration form and submit it to the National Infertility Unit (via fax at 1-860-607-7476). Medical deductible and plan costs will apply. (Your cost will depend on your individual plan.)
   • Infertility providers are responsible for treatment authorizations.
   • The REI Health Guide will contact the Express Scripts’ Client Service Team with the pre-certification approval.
